Additionally, I will send you the consultation form in Microsoft Word via email prior to our meeting to facilitate electronic completion.

 Following the consultation, I will develop a tailored treatment plan, which I will share with you within 48 hours. Upon your agreement to the proposed treatment, kindly sign the contract and return it to me via email. Again, for your information, a PDF copy of the contract can be downloaded below.
